Job Description:
A company is looking for a Behavioral Health Lead Population
Health Clinical Program Development Strategist.
Key Responsibilities
Lead and oversee the design of population health strategy
initiatives to improve care coordination and health outcomes
Monitor care management trends and analytics to support program
initiatives and identify high-risk member populations
Collaborate with senior leadership to provide insights for
improving coordination of care and overall health outcomes
Required Qualifications, Training, and Education
Bachelor's degree and 5-7 years of related experience, or
equivalent experience
Required clinical license (e.g., LCSW, LCPC, LICSW, Ph.D. or
Psy.D., PMHNP, CNS, LMHC, RN, Pharm.D)
Experience in designing clinical and population health programs
Expertise in managing clinical, quality, documentation, and data
submission projects
Ability to work cross-functionally to identify process
improvements
