Job Description:
A company is looking for a Clinical Pharmacist (Remote) to
support patients in promoting health and managing chronic
diseases.
Key Responsibilities
Lead a patient-centered, pharmacist-led disease management program
for chronic diseases
Collaborate with healthcare providers to ensure optimal patient
care
Conduct Medication Therapy Management (MTM) and Comprehensive
Medication Reviews (CMR)
Required Qualifications
Doctor of Pharmacy (PharmD) with a current pharmacist license in
good standing
Clinical experience in Medication Therapy Management
Experience in pharmacist-led Clinical Disease Programs (e.g.,
Diabetes, COPD)
Knowledge of healthcare industry practices and HIPAA
regulations
Ability to prioritize tasks and manage multiple projects
simultaneously
