direction, is responsible for providing the health status
assessment, maintenance, disease prevention and supervision of
comprehensive health care in collaboration with and under the
supervision of a physician. Provides services in accordance with
UCP of NYC policies, the standards of the profession and the
regulations and requirements of applicable regulatory
agencies.Essential Duties And ResponsibilitiesThe duties and
responsibilities of the Nurse Practitioner will include but are not
limited to the following:
- Obtains complete history of the people we support, recording
elements of past health history, family history, review of health
habits and environmental data required to diagnosis a condition and
render proper treatment.
- Identifies and documents patient issues appropriately.
- Ensure documentation on the record for the people we support is
complete.
- Participates in in-service education for other patient care
personnel relevant to program.
- Participates in interdisciplinary team meetings and annual
reviews as required by each program.
- Delivers service with effective and caring interpersonal
behavior.
- Orders and performs necessary tests to ensure proper diagnosis
and interprets test results.
- Develops a detailed plan of treatment, therapy and/or
rehabilitation and directs patient care team to implement.
- Assists patients and family members with identifying and
utilizing resources that may lead to his/her optimum health.
- Prescribes/administers medications, therapeutic diet,
immunizations, etc.
- Consults with other medical specialists to aid in diagnosis and
treatment. Initiates appropriate referrals as needed.
- Manages caseload effectively.
- Respects client's right to privacy and confidentiality.
- Explains information to patients and families in an
understandable manner.
- Serves as an active participant in quality assurance
program.
- Works with other disciplines to secure required services for
the people we support.
- Utilizes planning and problem-solving techniques with staff and
management.
- Adheres to program and agency policies and procedures.
- Keeps abreast of changes in field.
- Performs nursing duties in urgent circumstances.
- Performs other related duties as
requested.RequirementsQUALIFICATIONSTo perform this job
successfully, an incumbent must be able to perform each essential
duty satisfactorily. The requirements listed below are

- Ability to read, write, speak and understand English.
- Good interpersonal skills are required in order to interact
effectively with employees, co-workers, the people we support,
their families, other programs, outside contacts, etc.
- Appropriate delineation of clinical privileges.
- Due to sensitive nature of the position, must be able to
maintain the strictest confidentiality.
- Demonstrates energy, independence and ingenuity.
- Ability to work independently and handle multiple tasks.
- Good organizational skills.
- Ability to input and retrieve information from a personal
computer.
- Good oral and written communications skills.
- Good analytical ability to interpret client examination results
and properly administer required tests.EDUCATION And/or EXPERIENCE
- Master's Degree in Nursing.
- At least two years of prior experience as a Nurse
Practitioner.
- Must have a valid NY State Nurse Practitioner license.
- Certified for prescription privileges by NY State.
- Must be Board Certified; preferably in Family
Practice.COMPENSATION: $100-$125/hour (Based on experience)At
