DescriptionPurpose of Position:Under general supervision, serves as
a licensed, independent practitioner responsible for planning,
coordinating, delivering and managing comprehensive psychiatric and
mental health care services in a health care setting. Ensures
patient and family-centered quality care to improve outcomes and
patient experience, in accordance with applicable NYC Health +
Hospitals policies and procedures, state and federal rules and
regulations and nursing standards of care.Essential Duties And
Responsibilities
- Provides continuous comprehensive psychiatric and mental health
nursing care, including preventive and supportive health care
services to psychiatric patients and their families.
- Manages acute and chronic mental health problems and
coordinates health care for psychiatric patient population.
Identifies and diagnoses illnesses and physical conditions, and
implements therapeutic and corrective measures.
- Consistent with the established nursing practice,
differentiates between normal and abnormal findings in the
screening process to detect changes in health status that require
physician's intervention.
- Develops and maintains partnerships with community-based
organizations and makes referrals to consulting physicians and/or
other health care professionals and community agencies, as
necessary.
- Evaluates and interprets patient data, in order to plan and
implement appropriate nursing interventions.
- Consults with physicians and other health care practitioners in
the evaluation of health screening findings to make prospective
decisions about treatment that will best support improved outcomes
and patient satisfaction.
- Formulates differential diagnosis and the plan of care based on
medical history, physical exams, nursing assessment and other
pertinent information. Identifies health and medical risks and
needs.
- Develops, implements, and evaluates patient plan of care.
Determines effectiveness and modifies as per clinical standards and
practice protocols, where needed, to achieve medical and health
goals.
- Orders, conducts, and interprets diagnostic and laboratory
tests. Identifies and may prescribe appropriate pharmacological and
non-pharmacological interventions and therapies.
- Manages a patient session schedule allowing for both scheduled
and non-emergent unscheduled visits; maintains a work schedule that
ensures availability and continuity of care; ensures optimal
availability and utilization of appropriate resources.
- Provides education, guidance, and direction to psychiatric
nursing staff and other appropriate members of the health care team
to ensure quality and safe medical and nursing care is
provided.
- Provides education and counseling to psychiatric patients and
their families regarding health status, treatment and progress,
medication management, disease prevention and health promotion,
records and maintains a system of relevant documentation.
- Performs other related duties, as directed.Minimum
Qualifications
- A valid New York State license and current registration to
practice as a Registered Professional Nurse issued by the New York
State Department of Education (NYSED); and
- Current certificate as a Nurse Practitioner issued by the
NYSED; and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
