• You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education
• You will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients
• You will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers
  • Current NYS RN license and registration required
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of one year acute medical-surgical nursing experience required
  • Certified Home Health Agency (CHHA) experience preferred
  • Valid Driver License and Car
  • Bilingual English and Spanish or Mandarin or Cantonese preferredFee for Service Visit Rates:Start of Care Weekend/Holiday Visit Rate: $120 (Maintain the premium for 3+)Start of Care Weekday Visit Rate: $100 (Maintain the premium for 3+)Revisit Rate: $85Orientation Daily Rate: $250In-Service Hourly Rate: $55
