The Ambulatory Care Network is committed to improving their physical, behavioral, and social health and well-being to achieve healthcare equity for all. Within the Ambulatory Care Network Primary Care sites, the Integrated Mental Health in Primary Care Program (IMP) partners with primary care teams to support: Pre-visit clinical screening - like depression and substance use - for primary care patients, Education, consultation, and support for primary care providers around behavioral health issues, On-site time-limited behavioral health services in primary care, and referral for ongoing care, Linkages with community-based social service organizations. IMP administratively supports the infrastructure for clinical screening and provider education, and its clinical team provides culturally sensitive and compassionate mental health and substance use services to children, adolescents, and adults in their primary care clinic. Working as part of the care team, IMP psychiatric nurse practitioners, social workers, psychiatrists, case managers, and psychologists provide diagnostic and medication consultation, stabilization, referrals, and short-term talk therapy and medication management. Evidence-based models include Collaborative Care and SBIRT.Program Lead - Integrated Mental Health in Primary Care Program - ColumbiaUnder the guidance of the Program Director, the Program Lead will:

  • Assist in establishing program policies and procedures.
  • Track and document program performance, oversee tracking system and maintain database.
  • Coordinate logistics of meetings, site visits, and other program events.
  • Participate in the strategic planning process for the program.
  • Prepare and submit all program reports as required.
  • Update program face sheets, orientation guide, and other programmatic materials.Preferred Criteria
    • Master's degree
    • Bilingual in Spanish
    • Data Management SkillsRequired Criteria
      • Bachelor's Degree
      • Minimum of three (3) years of equivalent work experience
      • Excellent engagement and interpersonal skills and comfort with diverse communities
      • Highly organized and able to multi-task
      • Ability to work independently as well as collaboratively on a team
      • Working knowledge of Microsoft Office (Word, Excel, PowerPoint)
