HumanEdge Skilled Nursing and Allied Health is seeking a local contract Physical Therapist for a local contract job in Queens, New York. Job Description & Requirements

• Employment Type: Local Contract Key Responsibilities:
  - Assess & Treat: Conduct comprehensive evaluations and create individualized treatment plans.
  - Therapeutic Interventions: Provide physical therapy treatments, exercises, and modalities to enhance mobility and function.
  - Educate & Support: Teach participants and families about treatment plans and home exercises.
  - Document Progress: Maintain accurate records using EMRs like Care Compass.
  - Collaborate: Work closely with a multidisciplinary healthcare team to provide holistic patient care.
  - PACE Compliance: Ensure all care meets Program of All-Inclusive Care for the Elderly (PACE) regulations.
  - Home Visits: Travel as needed to participants' homes, nursing facilities, and other care sites.
  
  Qualifications:
  --- New York State licensure as a Registered Physical Therapist.
  --- 1+ years of experience (geriatrics preferred).
  --- Bi-lingual preferred (based on participant demographics).
  --- Physical ability to lift up to 50 lbs, transfer participants, bend, kneel, reach, and stand for extended periods.
  --- Strong interpersonal skills to build rapport with participants and staff.
  --- Knowledge of EMRs (Care Compass preferred).
