• Current RN Licensure in New York State
• BLS (If not, BLS is required before the start date)

  • Required Licensure- Current RN Licensure in New York State
  • Required Experience- Two (2) years of experience in a HOME setting within the last ten (10) years.
  • Ability to review health status questions and make evaluations and determinations based on UAS criteria for enrollment.
  • Ability to work with technology in the workplace.
  • Knowledge of the theory, principles, and practices of general professional nursing.
    • Job Requirements
    • Two (2) years of experience in a HOME setting within the last ten (10) years.
    • Experience conducting assessments similar to the UAS such as OASIS.
    • Providing primary care to homebound persons.
    • Care management for persons receiving home and community-based services including Hospice and Assisted Living.
      • Additional Details
      • All candidates being submitted must be under the impression they will be conducting hybrid Tele and in-field assessments.
      • Candidates can't do UAS assessment assignments elsewhere and need to sign the COI form before start.
