We are seeking a dedicated and compassionate Medical Provider to join our team as a Primary Care Provider (PCP). As a PCP at Callen-Lorde, you will play a vital role in delivering comprehensive care, making a positive impact on the lives of our diverse patient population.

• Screening, diagnosing, and treating Callen-Lorde patients.
• Providing care in a sensitive and non-judgmental manner.
• Making appropriate internal and external referrals.
• Offering comprehensive care based on pre-arranged schedules (15-20 min appointments)
• Documenting care promptly and following charting protocols (EHR: EPIC)
• Ensuring appropriate follow-up for ordered care, including referrals and test results.
• Providing care to patients living with HIV and Trans individuals. Medical providers new to HIV and Trans care will participate in didactics, precepting, and case conferencing in order to build competency in HIV and Trans care.
• Contributing to continuous quality improvement efforts.
• Maintaining familiarity with clinical guidelines in various areas of care.
