Job Description:
Overview Oversees, supervises, coordinates, facilitates and
supports Nurses and extended Care Team. Provides clinical and field
supervision of assessments and care coordination interventions,
including but not limited to, Telehealth, Health Risk Assessments
and post-discharge assessments. Supervises the day-to-day clinical
and operational health related technology (i.e. Telehealth and
other web-based applications) as a standard of care, integrating
virtual data in clinical decision-making and care delivery
processes. Ensures assessments and care coordination is provided
with the standards of nursing practice. Engages in the development,
refinement, and didactic content necessary to support clinical
decision making. Works under general supervision.--- Ensures the
assessments, health coaching and support, as well as care
management services, provided to patients/families of VNSNY
Population Health and its contractual partner's meet the medical,
physical, and emotional needs of the patient and family. --- Based
on the team this job supervises, this will include either providing
effective patient assessments and care coordination services or
providing effective triaging of new assessments, Telehealth and
facilitating implementation of new initiatives. --- Ensures
workload assigned is appropriate to nurse's ability in order to
provide quality and comprehensive services; provides clinical
oversight of nurses through direct observation (home visit; call
auditing) and indirect methods (i.e. chart review, review of
written work, and feedback from staff). --- Evaluates clinical
assessments for compliance with VNS Health Population Health
policies and procedures and contractual services. --- Participates
in business planning, financial and clinical analysis of the
department's programs, projects and initiatives --- Collaborates
with Quality/Education in the development and implementation of
quality improvement and education activities to ensure professional
nursing practice standards are consistently met. Utilizes a variety
of methods and resources to identify and address individual
learning needs of clinicians. Promotes critical thinking and
communication skills as a role model for professional nursing
practice. Collaborates with Quality and Education staff to design,
implement, evaluate, and modify quality initiatives and education
programs as needed. --- Facilitates the creation education
curricula for Population Health Management and Telehealth
integration into care processes. Collaborates across the
organization with managers and educational resources to ensure
staff awareness of Population Health Management, and competency
with health related technologies. Participates in agency efforts to
inform internal and external customers of the technology assisted
care management programs. --- Maintains current knowledge of
clinical assessments and health technology industry offerings,
trends in service delivery and future opportunities for product
development. Participates in appropriate professional organizations
and vendor forums. Represents VNS Health as a leader in technology
assisted care as required. --- Performs all duties inherent in a
supervisory role. Ensure effective staff training, interviews
candidates for employment, evaluates staff performance and
recommends hiring, promotions, salary actions, and terminations, as
appropriate. --- Participates in special projects and performs
other duties as assigned. Qualifications Licenses and
Certifications:
License and current registration to practice as a Registered
Professional Nurse in the State of New York required New York State
Certification as a Nurse Practitioner preferred Telehealth Nursing
or Care Management Certification preferred
Education:
Master's Degree in Nursing required
Work Experience:
Minimum of two years of clinical experience in an Assessment or
Hospital setting required Demonstrated expertise in clinical care
management required Experience in healthcare management preferred
Clinical expertise and critical thinking skills required Ability to
advise patients and colleagues on appropriate health related
technology usage required Effective oral, written and interpersonal
communication skills required Knowledge of personal computer
operations, including MS Office applications required Travel to
offsite locations may be required based on operational needs.
