This includes diagnosing and treating common conditions, educating patients in developing skills and knowledge to control chronic illness, promoting preventive health through lifestyle modification and recommending preventive services. NPs order and review diagnostic tests and ensure access to timely and appropriate levels of care (e.g. same day appointments). NPs coordinate patients' care across various locations of service (e.g. emergency, primary, specialty, dental, mental health, substance use, inpatient and at home care).The focus of this role is supporting Surgical and Medical Sub-specialty providers to provide timely access for patients seeking standardized surgical or specialty care evaluation and treatment.Minimum Qualifications

• Valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Education Department (NYSED); and
• Current certification as a Nurse Practitioner issued by the NYSED; and
• Current certificate issued by the NYSED to prescribe medication; and
• Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
• Two (2) years of satisfactory experience as a Nurse Practitioner, or two (2) years of satisfactory experience as a Registered Professional Nurse, eighteen (18) months of which must be as a Nurse Practitioner preferred.For Assignment To Nurse Practitioner (Primary Care/Behavioral Health)
