Job Description:
A company is looking for a Transition Specialist to support
Medicaid transition of care for members in the community.
Key Responsibilities
Support care coordination teams and providers in developing
effective transition plans for members
Assist with member transitions based on enrollment or care service
needs
Track transition metrics and ensure proper documentation of care
transitions
Required Qualifications
Bachelor's degree required
2 - 4 years of related experience preferred
Equivalent experience may be considered
Knowledge of healthcare services and transition planning
Experience in coordinating care and working with healthcare
providers
