Responsibilities
A day in the life of a Patient Placement Coordinator at Hackensack Meridian Health includes:Collaborates with Capacity Manager(s), nursing supervisors/Directors, infection control staff, ED leadership, EVS & Transport leadership to ensure efficient execution of patient placement decisions to ensure patient safety. Maintains ongoing communication with Capacity Managers & aforementioned staff to maintain continual patient flow and ensure essential personnel are informedUtilizes appropriate tools and technology to monitor and promote continuous patient movement. Ensures accurate admission, discharge, and transfer data and all patient flow measures and statistics as defined by Capacity Management teamDemonstrates expertise in working with required computer applications, phone systems and patient tracking toolsInteracts with all required stakeholders (nursing, physicians, EVS, transport, leaders and team members, etc) utilizing appropriate communication skills and tools. Performs daily work tasks maintaining extraordinary Customer Service at all timesOther duties and/or projects as assignedAdheres to HMH organizational competencies and standards of behavior
Qualifications
High School diploma, general equivalency diploma (GED), and/or GED equivalent programs requiredKnowledge and experience with computers, OS usage, Outlook, Word, Excel and basic keyboarding skills requiredMinimum of 2 years in a medical/acute hospital setting required If you feel that the above description speaks directly to your strengths and capabilities, then please apply today!
