DescriptionPurpose of Position:Under general supervision, serves as
a licensed, independent practitioner responsible for planning,
coordinating, delivering, and managing comprehensive health care
services in a health care setting. Ensures patient and
family-centered quality care to improve outcomes and patient
experience, in accordance with applicable NYC Health + Hospitals
policies and procedures, state and federal rules and regulations,
and nursing standards of care.Examples Of Typical Tasks
- Provides continuous comprehensive medical and nursing care,
including preventive and supportive health care services to
patients and families.
- Manages health problems and coordinates health care for various
patient populations. Identifies and diagnoses illnesses and
physical conditions, and implements therapeutic and corrective
measures.
- Consistent with the established nursing practice, differentiate
between normal and abnormal findings in the screening process to
detect changes in health status that require physician's
intervention.
- Develops and maintains partnerships with community-based
organizations and makes referrals to consulting physicians and/or
other health care professionals and community agencies, as
necessary.
- Evaluate and interpret patient data, in order to plan and
implement appropriate nursing interventions.
- Consult with physicians and other health care practitioners in
the evaluation of health screening findings to make prospective
decisions about treatment that will best support improved outcomes
and patient satisfaction.
- Formulates differential diagnosis and the plan of care based on
medical history, physical exams, nursing assessment, and other
pertinent information. Identifies health and medical risks and
needs.
- Develop, implement, and evaluate patient plan of care.
Determines effectiveness and modifies as per clinical standards and
practice protocols, where needed, to achieve medical and health
goals.
- Orders, conducts and interprets diagnostic and laboratory
tests. Identifies and may prescribe appropriate pharmacological and
non-pharmacological interventions and therapies.
- Manages a patient session schedule allowing for both scheduled
and non-emergent unscheduled visits; maintains a work schedule that
ensures availability and continuity of care; ensures optimal
availability and utilization of appropriate resources.
- Provide education, guidance, and direction to nursing staff and
other members of the health care team to ensure quality and safe
medical and nursing care is provided.
- Provides education and counseling to patients and families
regarding health status, treatment and progress, medication
management, disease prevention, and health promotion, records, and
maintains a system of relevant documentation.
- Performs other related duties, as directed.Neurology Nurse
PractitionerThe Neurology Nurse Practitioner is a specialized
healthcare professional who works collaboratively with neurologists
and other healthcare staff to provide comprehensive care to
patients with neurological disorders. This role involves assessing,
diagnosing, and treating patients with conditions such as stroke,
epilepsy,multiple sclerosis, and other neurodegenerative diseases.
The practitioner also educates patients and their families about
neurological conditions and treatment options.Patient Assessment:
Conduct detailed neurological examinations to assess cognitive,
sensory, and motor functions. Use tools such as the Glasgow Coma
Scale or NIH Stroke Scale for assessments.Diagnosis and Treatment:
Collaborate with neurologists to diagnose neurological conditions.
Develop and implement treatment plans, which may include
prescribing medication, ordering physical therapy, or referring to
specialist services.Patient Management: Monitor the ongoing
treatment and rehabilitation of patients with chronic neurological
disorders. Adjust treatment plans as necessary.Patient Education:
Provide patients and their families with information about their
conditions, treatment options, and lifestyle adjustments to manage
symptoms.Research and Continuous Learning: Stay updated with the
latest advancements in neurology and incorporate evidence-based
practices into patient care.Interdisciplinary Collaboration: Work
closely with other healthcare professionals, including physicians,
physical therapists, and social workers, to provide holistic
care.Emergency Response: Recognize and respond to neurological
emergencies, such as stroke or seizure, providing immediate care
and coordinating with emergency services.Minimum Qualifications
- A valid New York State license and current registration to
practice as a Registered Professional Nurse issued by the New York
State Department of Education (NYSED); and
- Current certificate as a Nurse Practitioner issued by the
NYSED; and
- Holds, or obtains through facility orientation, a valid and
current certification in Basic Life Support (BLS) through the
American Heart Association (AHA).Department PreferencesDepartmental
Preference: Strongly prefer National Board Certification in the
