• Pet care discount 25% with participating ProvidersJob DetailsWe are looking to hire an experienced Nurse Practitioner to provide direct skilled care to patients in need of Palliative Care.A background in Hospice or Palliative Care is preferred but not required. Oncology experience is desiredThis full time role has coverage 3 days per week in Brooklyn where you will see patients in the SNF, Rehab and community settings.1 day per weeek is remote / work from hom as you will have a Team IDT meeting virtually from 9am to 12 noon and will spend rest of th day doing telehealth / virtual visits with your patints.The fifth day will be onsite in Manhattan at a SNF only doing Palliative Care consults.NP should have at least 2 years experience in either Hospice, Palliative Care or Oncology or Geriatrics to be considered.The Nurse Practitioner will provide direct skilled nursing care to patients in thenursing home, hospital or community setting and will manage pain and symptom management,communication including goal setting discussions, and hospice eligibility review.
  • NP will do patient assessment - both cognitive and physical, and may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family.
  • NP will be responsible for taking patient history, do appropriate examination / patient assessment, create and implement a plan of care to prevent or mitigate illness and not burden for the patient and family.
  • Orders appropriate diagnostic testing
  • Formulates a problem list
  • Recommends interventions consistent with a care plan that advances the objectives of palliative
  • care
  • Provides interventions as appropriate, consistent with a care plan that advances the objectives of
  • palliative care
  • If appropriate, takes the lead in implementing key elements of palliative care, including family
  • meetings, goal setting discussions, and symptom management.
  • Performs hospice eligibility review and engages in accurate hospice-related discussions
  • Collaborates with patients' physicians and other treatment team members
  • Evaluates patient's response to treatments, modifies or changes when needed.
  • Provides patient and family education regarding the disease process and end of life journey
