Job Title: Licensed Practical Nurse

Pay Rate: $38 - $40/HR

Schedule: 5x8 with rotating weekends
--- Days: 7:00 AM - 3:00 PM
--- Evening: 3:00 PM - 11:00 PM
--- Overnight: 11:00 PM - 7:00 PM

Requirements:
--- Graduation from an approved program in professional nursing(LPN).
--- Current NYS LPN Licensure
--- AHA BLS Certificate
--- Minimum (2) years of LPN experience
--- Hospital med-surg or nursing home experience preferred

Duties:
--- Assist with daily living activities and mobility.
--- Administer medications and provide wound care.
--- Develop and implement care plans, and check and record vital signs.
--- Conduct health assessments and observe and report health changes.
--- Educate patients and families on health conditions.
--- Maintain patient records and ensure regulatory compliance.
--- Manage supplies and equipment.
