Job Description:

A company is looking for an RN Case Manager to support members with complex medical and behavioral needs remotely in Arizona.

Key Responsibilities

Engage members to complete comprehensive needs assessments covering medical, behavioral, and socioeconomic factors
Develop and implement person-centered care plans addressing chronic health conditions and promoting wellness
Collaborate with internal teams and community resources to execute care plans and advocate for members' needs

Required Qualifications

Current, unrestricted independent licensure as a Registered Nurse in Arizona
4+ years of clinical experience
1+ years of experience with MS Office applications
Residency in Arizona
